"Trouble came to the world through laziness"
I can understand know why it is called that Medicine is a combination between art and science. It is not just like math that..if we calculate 2 times 2 then it will be equal to 4. If 2 doctors are compared to treat a patient with a specific disease, so from the beginning he examines the patient we can know that the element of the art influences the way the patient is treated.
It is just like a sun..we don’t always give a yellow color when we want to color it. We can give yellow, or orange or black as we want depend on the picture we make. So that with the patient..we can’t always give the same treatment for the same disease..we should consider his other accompanying disease, social status,family who will support him in the hospital, the benefit and the risk if we choose the drugs and anything else that come from inside his body( Of course it is not always the same). I wrote this cause this baby asked to be treated by his parent at house without any equipment and experience. It passed the first death test and now are facing the 2ndone..but his parent gave Up. I motivated him to be patience cause it was not easy to treat a newborn baby. But he still insists to be back home cause of the limited money. I’m so disappointed cause he chose a put his baby, the first one, at stake and (again) facing a deadly disease without any supporting medical equipment. This baby still can’t suck and is in an active disease..
This is the story….
One baby, delivered by a mid wife in nearest city from Serukam, got a very severe difficulty of breathing because this baby was still in the passage of delivery although the membrane had been ruptured for 4 days. The mid wife just said it was ok and she observed this mom till there was a full dilatation of the cervix. Can u imagine this baby had no protection inside the hip/pelvis for 4 days? Can u imagine that inside the vagina there are so many bacteria which could go inside this baby?
4 days later, this baby delivered and after that it won’t cry for almost 4 hours. And his father still waited at the mid wife to see his son breath normally for 4 hours..( how many oxygen deficit that this son had?). Then, in the ER room, I was there alone cause there was no patient in the ward. Actually, I really felt so incapable for this baby cause I think I think that he was at a condition that all of the bacteria spread inside all over his body that we call it SEPSIS, his blood sugar was 27mg/dl, a bit blue, couldn’t breath and so cold…
So at that I explained to his father that his condition was really bad (the oxygen saturation was 78% with the Oxygen from his nose) So I asked the nurse to do intubation so this baby could have a better oxygenation. But if in 2 days, it still used this ventilator to help breathing, I suggested to go to Pontianak to have a better ventilator.
Amazingly, he could make it. In 2 days he started to breath spontaneously and on the seventh day he can be treated in the regular ward without the tube. It was in ICU before. He could make it!! I’m so amazed cause this is my 1st time, I did extubation for my patient..and It was alive.. Before, every patient who use the ventilator..I just wait for the patient’s time to pass away. I’m bit traumatized to help treating patient who has terminal or very bad condition. I really give up and couldn’t think any further for this baby condition. SO I really thanks to GOD, He really helped him in his critical state. My first patient who can be saved from the Ventilator… THANKS GOD..!!
The problem with this newborn baby now is he got aspiration pneumonia when it tried to suck. His parent want to be back home immediately although his son still couldn’t suck. So right now, this patient is facing another critical condition..but his parent chose to give up..!! Facing the human’s behavior is art that never been taught…
